Subject: Appointment of HIPAA Privacy and Security Officers
To comply with the HIPAA Privacy Rule (45 CFR §164.530) and HIPAA Security Rule (45 CFR §164.308(a)(2)), Control Alt Growth hereby formally designates individuals responsible for overseeing the organization's privacy and security compliance programs relating to the File Logic platform.
1. Designated HIPAA Privacy Officer

Responsibilities:
- Oversees compliance with the HIPAA Privacy Rule
- Maintains privacy-related policies and procedures
- Responds to client inquiries regarding privacy practices
- Coordinates privacy compliance efforts across the organization
- Participates in breach notification and incident review processes
2. Designated HIPAA Security Officer
Name: Bridger Tower
Role: Security Officer
Responsibilities:
- Oversees compliance with the HIPAA Security Rule
- Manages technical safeguards in File Logic (access controls, encryption, logging)
- Conducts regular security reviews and risk assessments
- Oversees incident response for security-related events
- Coordinates with engineering to ensure HIPAA-aligned architecture
3. Duration & Authority
These appointments are effective immediately and remain in effect until modified in writing by Control Alt Growth leadership. Officers are authorized to enforce policies, oversee training, conduct audits, and escalate issues to leadership as necessary.
4. Acknowledgement
By designating these officers, Control Alt Growth affirms that it maintains governance structures appropriate for handling Protected Health Information (PHI) within the File Logic platform.
